HYPERTENSION IN PREGNANCY

 

-Methyldopa is recommended for females with hypertension that is first diagnosed in  pregnancy. 

 

-Hydralazine is the IV drug recommended for severe hypertension during pregnancy.  It is also used for the treatment of eclampsia and pre-eclampsia.

 

-Diuretics are indicated for the treatment of chronic hypertension if they are prescribed before the start of conception.  Diuretics are also indicated in salt-sensitive pregnant females.

 

-Beta-blockers such as atenolol, metoprolol, and labetalol have been shown to be safe in late pregnancy, but may cause fetal growth retardation when used in early pregnancy.
-Calcium blockers may produce hypotension if given with magnesium to treat pre-eclampsia and/or eclampsia

 

-ACE inhibitors, and Angiotensin II receptor blockers are contraindicated in pregnancy since they have been associated with fetal death and renal failure during the last two trimesters of pregnancy.
