COCAINE AND HYPERTENSION
-Coronary ischemia due to cocaine should be
treated with IV nitroglycerin. Nitroprusside
should be used to treat cocaine induced severe hypertension not associated
with coronary ischemia.
-Beta-blockers should be avoided for the
treatment of cocaine induced hypertension since these drugs may worsen the
hypertension and/or cause coronary ischemia due to catecholamines acting on
the alpha receptors that are
unblocked. Labatolol has
alpha and beta blocking properties and appears inferior to IV nitroglycerin
or Nitroprusside in the management hypertension due to cocaine.
