PHEOCHROMOCYTOMA AND HYPERTENSION

-IV phentolamine (alpha 1 and alpha 2 receptor blocker)  is indicated to treat severe HTN secondary to pheochromocytoma.  Beta-blockers should only be used after phentolamine has blocked the alpha receptors.  If  the beta-blocker is given prior to starting phentolamine,  the beta-blocker may worsen the hypertension and/or cause coronary ischemia due to catecholamines acting on the  alpha receptors that are unblocked.
